Process for Sample Submission Forms (SSF): All samples sent to Nelson Labs for testing must be accompanied by a complete and signed Sample Submission Form. The signature represents you agree to our Service Terms and Conditions. Effective 04 February 2019, samples received without a signed SSF will be held without testing until a signature for services is sent to Nelson Labs. Please contact your account manager with any questions.
At this time, the Secure Customer Portal is available for customers of the U.S. lab locations only (Salt Lake City, UT; Itasca, IL; Ontario, CA).
