Online Sample Submission Form Errors

20 Jul 2026 — We are currently experiencing an issue that is preventing customers from completing the online Sample Submission Form (SSF) through the customer portal. Please download the SSF and once completed, print and sign the form at the bottom, then include a hard copy of the signed form in the shipment containing your samples.

Please be aware that this secure site will store a limited amount of report data.  Data is archived on a regular basis and it is important that you download your final reports.  If you need report data that you do not see on our secure site, the Service Center can help you find what you need.  Please email us at servicecenter@nelsonlabs.com with your request.
